solution to transfering from a patio drive - grass/garden

im starting a project sorting out my mums area behind her house but the main issue is blending the sperate bits as the drive stops and then theres a bit of concrete then grass and im not sure what to do to blend the patio with the grass if anyones got any thoughts on the matter i would be much appreciated.
