clay soil is very fertile just add organic matter or some of your gravel!to improve the drainage worked wonders for me, garden is happilly...
sounds like you have applied to much ..i work on bowling greens and if you spill fertilizer the area goes black ,,it will eventually recover but...

probally caused by different grass species in turf or seed mix,,personally i would not worry about it ,it will blend in over time
weeds are easy to get rid off .if you dont mind using a selective weedkiller! poa is a pain most lawns have them,,i would thought it vey unlikely...
